SK Form Builder can send a Shopify form submission to Klaviyo by creating or updating a Klaviyo profile and adding that profile to a list you choose. The catalog documents that connection. It does not publish a field map, and it does not assign Klaviyo to a paid tier.

Use this when the form should feed a list your team already mails from Klaviyo. If you only need a short email popup inside Shopify marketing, Shopify Forms is the simpler tool. Shopify’s forms help, as used for this campaign, describes Shopify marketing automations. It does not describe this Klaviyo step. The Klaviyo step below is the SK Form Builder catalog, on the SK Form Builder page.

The quote app is a different product. Its catalog does not include Klaviyo. Do not point a quote form at these instructions.

What you want in Klaviyo

Decide the list before you connect the form. A contact form, a wholesale application, and a post-purchase survey should not share one list if the emails you send them are different.

  • Profile: the app creates or updates a Klaviyo profile from the submission.
  • List: it adds that profile to a list you select in the app.
  • Not in the catalog: a field-by-field map of form questions onto Klaviyo properties. Check one test profile before you name a property in the form label.

Email is the identifier you should require on any form that syncs. The catalog does not say a nameless, emailless submission becomes a usable profile. Make email required, and use the email field type so the address is validated.

Other connectors exist on this same form app: Mailchimp, Google Sheets, HubSpot, ActiveCampaign, Airtable, Slack, Zapier, and Shopify Flow. HubSpot on the form app creates a CRM contact and adds it to a list. That is not a Klaviyo feature, and it is not a connector on the quote app. Turn on only the destination you tested.

There is no verified plan gate

Starter is free. Basic is $4.99 a month or $49.90 a year. Pro is $12.99 a month or $129.90 a year. The catalog lists Klaviyo beside those prices without saying which tier includes it. It does not say Starter is excluded, and it does not say Klaviyo starts on Basic. Confirm the connector on the plan you are buying.

The same warning applies to Google Sheets and to CSV export. Sheets can append a row, with a header written the first time. CSV, Excel, and PDF export exist. Large CSV exports over 1,000 rows are emailed as a download. Those are backups if you do not want a live Klaviyo list. They are not the Klaviyo sync.

Fields, consent, and what not to assume

Collect the address you are allowed to mail, plus the facts the flow needs. A wholesale form should not silently join a consumer newsletter. Put a separate checkbox on the form, with your own consent sentence. Conditional logic can show that checkbox, or a second list’s questions, only when a choice equals “email me.” Operators for that rule are equals, contains, is empty, and greater or less than.

The catalog does not say the Klaviyo connector stores a consent timestamp, a double opt-in flag, or a map of custom properties. If your list requires double opt-in, confirm that behavior in Klaviyo with one test profile. Do not describe it as an SK Form Builder setting unless you see it in the app.

Smart prefill can fill name and email from a logged-in Shopify customer. A logged-in buyer is not the same thing as a person who agreed to a campaign. Keep the consent checkbox even when prefill is on.

A safe first form

Use three fields: email, first name, and one checkbox whose label is the consent line your policy already uses. Connect that form to one empty Klaviyo list. Submit it yourself. Then add more questions only after that profile looks right.

Test one subscriber before you announce the list

Create a list in Klaviyo that is not your main newsletter, or use a segment you can delete. Submit the Shopify form once, from the storefront, with an inbox you control.

  • Find the profile in Klaviyo and check the email spelling.
  • Confirm it is on the list you selected, not on a list you did not select.
  • Submit a second time with the same email and see whether you get a duplicate profile or an update. The catalog says create or update. Confirm which fields changed.
  • Submit with the consent box unchecked, if the box is optional, and confirm you are not mailing that person.
  • Check the SK Form Builder submission as well. The Klaviyo sync does not replace the inbox, the CSV export, or the ticket that is created when the submission has a customer email.

A ticket is created for a submission that includes an email, with an SLA due date. That is support tracking. It is not a Klaviyo campaign event.

The form stores UTM source, medium, and campaign on the submission, and a Shopify Flow trigger can carry that submission data plus the UTM parameters. The Klaviyo line in the catalog does not mention UTM. Whether those values reach the Klaviyo profile is unverified. No production list was used to test it. Until a profile from a tagged URL shows the values, do not describe a UTM mapping.

Spam controls still run: honeypot, IP blocklist, reCAPTCHA v2 or v3, or hCaptcha, and a default of 10 submissions per IP each hour. A bot that gets through can still create a profile. The test list is how you notice that before a real campaign.

Questions about Shopify forms and Klaviyo

Does the free Starter plan sync to Klaviyo?

The catalog does not say. Klaviyo is documented, and the three prices are documented, without a feature matrix. Check the plan screen in the app.

Can a wholesale form skip the newsletter list?

Yes, if you connect that form to a different list, or you leave Klaviyo off on that form. The catalog says you choose the list. It does not say every form on the store shares one list.

Is there a published field map?

No. The verified behavior is create or update a profile and add it to a chosen list. Map extra properties only after you see them on a test profile.

Connect the form in SK Form Builder after the test profile matches the list you intended. Install SK Form Builder for that job, and keep Shopify Forms if all you needed was a marketing popup.
