A Shopify contact form can sit in a theme app block on a page, in a popup, or in a slide-in panel. Shopify’s own Shopify Forms app is the simpler fit for a short lead form. SK Form Builder is the fit when the form needs a larger field set, a file upload, or questions that change based on the answer.

You do not need to edit theme code to place the block. Add the app, open the theme editor, and put the app block on the contact page. Then submit one real test from the storefront before you announce the address.

The catalog for SK Form Builder lists three plan prices: Starter at no charge, Basic at $4.99 a month or $49.90 a year, and Pro at $12.99 a month or $129.90 a year. It does not say which fields belong to which plan. Treat every field below as available in the product, and confirm the plan you buy in the app before you promise a field to a customer.

Decide what the contact form has to collect

A useful contact form asks only what someone on your team will act on. Name, email, and a message cover most store questions. Add a phone field when support actually calls people back. Add an order number when the form is for existing buyers, not for first-time visitors.

SK Form Builder documents 34 input components. For a contact page, the practical set is smaller:

  • Text and email: name and a validated email address.
  • Telephone: a phone field with a country dial-code list.
  • Textarea: the message itself.
  • Select or radio: a short reason, such as order help, wholesale, or product question.
  • File upload: only if the shopper must attach a photo or a document. The catalog includes an extension allowlist. It does not publish a megabyte cap.

Smart prefill can fill name, email, or address from a logged-in Shopify customer, or from a URL parameter. That helps a returning buyer. It does not replace a required email check.

Two emails can go out on each submission: one to the merchant, with reply-to set to the customer, and an optional autoresponder to the customer. Both use merge tags for the customer name, the form name, the date, and the field values. Custom SMTP is available if you want mail to leave through your own server. If that server fails, the app falls back to its own sender and tells the merchant.

Use Shopify Forms when the job is a simple lead form

Shopify Forms, documented in Shopify’s forms help, is built for popup and inline forms tied to Shopify marketing. Use it when you want a name and email, a discount, or a customer tag, and you do not need a long custom form.

That is the right default for a newsletter popup or a short “contact us” block that only collects an email and a note. SK Form Builder’s catalog does not include Shopify’s Shop claim button. Do not describe SK Form Builder as a replacement for that Shopify Forms feature.

If the contact page later needs conditional questions, a file, or a draft order, keep Shopify Forms for the popup and add the other form only for that job. One store can use both.

Use SK Form Builder when the contact page needs more than a lead

The form renders through a theme app extension inside a closed shadow DOM. Theme CSS does not leak into the form, and the form’s CSS does not leak into the theme. You can show it inline, as a popup, or as a slide-in panel. The form definition is stored in a theme metafield, so the page does not fetch the whole schema on every view.

Place the block on the contact page in the theme editor, or on a page you create for wholesale and support. Check the block on a product page only if you truly want the form there. A contact form on every product template is easy to miss and hard to support.

What happens after someone submits

The app checks required fields, email, URL, and phone format in the browser, then checks again on the server. Email-domain allow and block lists run on the server. Spam controls include a honeypot, an IP blocklist, reCAPTCHA v2, reCAPTCHA v3, and hCaptcha. The default rate limit is 10 submissions from one IP each hour. Turnstile is not in the catalog.

A submission with a customer email becomes a support ticket, with an SLA due date and the message stored as the first note. Staff reply from a separate ticket portal. That portal login is not the merchant’s Shopify login. The catalog does not assign the ticket system to a paid tier.

The app can also look up or create a Shopify customer, apply tags, and write customer metafields from the submission. Use that when the contact form should update the customer record. Skip it when the form is only a message to support.

Test the form before you publish the page

Submit the form yourself from the live theme, not only from the app preview. Use a real inbox you can open.

  • Confirm the merchant email arrives and that reply-to is the address you typed.
  • Confirm the customer autoresponder, if you turned it on, arrives once.
  • Open the submission in the app and check every field you marked required.
  • Try the form on a phone. The shadow DOM keeps theme CSS out, so a desktop preview is not enough.
  • If you enabled captcha, submit once with it and once after you temporarily expect a bot-like repeat from the same IP, so you know the hourly limit behaves.

The storefront form ships with 34 locale files. The admin is translated separately. A translated storefront does not translate the sentences you write into the field labels. Write those labels in the language your buyers use.

Questions about a Shopify contact form

Can I add a contact form without editing theme code?

Yes. SK Form Builder uses a theme app block. You place that block in the theme editor. You do not paste a script into the theme for the basic embed.

Which fields are on the free plan?

The catalog names a free Starter tier and two paid prices. It does not assign individual fields to those tiers. Check the plan screen in the app before you tell a customer that a field is included.

Should every store replace Shopify Forms?

No. Keep Shopify Forms for a simple popup or inline lead form. Add SK Form Builder when the contact page needs the larger field set, a file upload, or conditional questions.

Read the field list on the SK Form Builder page, then install SK Form Builder only if that page matches the contact form you need. For a short email capture, start with Shopify Forms instead.
